About Sildenafil Citrate

Indications of Sildenafil Citrate

Sildenafil Citrate is indicated for the following uses:

Established / FDA-approved uses

  • Erectile dysfunction (ED): Treatment of erectile dysfunction in men, to help achieve and maintain an erection sufficient for sexual activity. Sexual stimulation is required for Sildenafil Citrate to be effective.
  • Pulmonary arterial hypertension (PAH): Treatment of PAH (WHO Group I) in adults, to improve exercise ability and delay clinical worsening. This use employs a separate formulation and dosing schedule from the erectile dysfunction indication (see Dosage and Administration).

Sildenafil Citrate is not indicated for use in women, and it is not a treatment for HIV or other sexually transmitted infections. It does not increase sexual desire on its own and has no effect in the absence of sexual stimulation.

Dosage & Administration of Sildenafil Citrate

Dosing by indication

IndicationAdult doseNotes
Erectile dysfunction50 mg taken as needed, approximately 1 hour before sexual activity (may be taken 30 minutes to 4 hours before); range 25-100 mg based on effectiveness and tolerabilityMaximum recommended dosing frequency is once per day
Pulmonary arterial hypertension20 mg three times daily, taken approximately 4-6 hours apartUses a separate, lower-strength formulation than the ED indication; with or without food

Dose adjustment for erectile dysfunction

Starting dose is usually 50 mg; the dose may be increased to 100 mg or decreased to 25 mg depending on response and tolerability. In patients taking potent CYP3A4 inhibitors (e.g. ritonavir, ketoconazole, itraconazole) or alpha-blockers, or in those with hepatic or severe renal impairment, a lower starting dose (25 mg) is recommended (see Interactions and Use in Special Populations).

Renal impairment

No dose adjustment is generally needed in mild-to-moderate renal impairment; in severe renal impairment (creatinine clearance <30 mL/min), a starting dose of 25 mg should be considered for erectile dysfunction.

Hepatic impairment

In hepatic impairment (e.g. cirrhosis), a starting dose of 25 mg is recommended for erectile dysfunction, as clearance of Sildenafil Citrate is reduced.

Side Effects of Sildenafil Citrate

Common and serious side effects associated with Sildenafil Citrate include:

Common

  • Headache
  • Flushing
  • Dyspepsia (indigestion)
  • Nasal congestion
  • Dizziness
  • Visual disturbances (e.g. altered color perception, blurred vision, light sensitivity)
  • Back pain and myalgia (more often reported with the PAH dosing regimen)

Uncommon to rare but serious

  • Priapism — a prolonged, painful erection lasting more than 4 hours; a medical emergency requiring immediate attention (see Precautions and Warnings).
  • Sudden decreased or loss of vision in one or both eyes, including non-arteritic anterior ischemic optic neuropathy (NAION) (see Precautions and Warnings).
  • Sudden decreased or loss of hearing, sometimes with tinnitus and dizziness (see Precautions and Warnings).
  • Cardiovascular events (e.g. myocardial infarction, chest pain, palpitations, tachycardia, and rarely sudden cardiac death) reported in temporal association with sexual activity in men with underlying cardiovascular risk (see Precautions and Warnings).
  • Hypotension, particularly when combined with interacting medicines (see Interactions).
  • Hypersensitivity reactions, including rash and, rarely, angioedema.

Contraindications of Sildenafil Citrate

Sildenafil Citrate is contraindicated in the following situations:

  • Known hypersensitivity to Sildenafil Citrate or any component of the formulation.

Frequently Asked Questions about Sildenafil Citrate

What is Sildenafil Citrate used for?

Sildenafil Citrate is used mainly to treat erectile dysfunction (difficulty achieving or maintaining an erection) in men. A different, separate dosing regimen of Sildenafil Citrate is also used to treat pulmonary arterial hypertension, a type of high blood pressure in the lungs. It does not increase sexual desire and only works with sexual stimulation.

How should I take Sildenafil Citrate for erectile dysfunction?

For erectile dysfunction, Sildenafil Citrate is usually taken as a single dose of 50 mg about 1 hour before sexual activity, though the dose may range from 25 mg to 100 mg depending on how you respond and tolerate it. It can be taken with or without food, but a heavy, high-fat meal may slow down how quickly it works. Do not take more than one dose of Sildenafil Citrate in 24 hours.

Can I take Sildenafil Citrate if I use nitrate medicines or nitroglycerin for chest pain?

No. Sildenafil Citrate must never be taken with nitrate medicines (such as glyceryl trinitrate/nitroglycerin, isosorbide mononitrate or dinitrate) in any form, or with riociguat, because the combination can cause a severe, potentially life-threatening drop in blood pressure. Tell every doctor and pharmacist that you take Sildenafil Citrate so nitrates are never prescribed together with it, including in an emergency.

What if I get an erection that will not go away after taking Sildenafil Citrate?

An erection lasting longer than 4 hours (priapism) is a medical emergency after taking Sildenafil Citrate. Seek immediate medical attention, as a prolonged, untreated erection can permanently damage the penis and cause lasting loss of sexual function.

Are there serious side effects I should watch for with Sildenafil Citrate?

Most people tolerate Sildenafil Citrate well, with headache, flushing, indigestion, and nasal congestion being the most common effects. Rarely, Sildenafil Citrate has been associated with sudden vision loss in one or both eyes, sudden hearing loss, and cardiovascular events such as chest pain or heart attack, particularly in men with underlying heart disease. Stop taking Sildenafil Citrate and seek immediate medical attention if you experience sudden vision or hearing changes, chest pain, or a prolonged erection.

Is it safe to combine Sildenafil Citrate with blood pressure medicines or alcohol?

Sildenafil Citrate can lower blood pressure further when combined with alpha-blockers or other blood pressure medicines, and excessive alcohol can add to this effect and to side effects like dizziness or flushing. Tell your physician about all blood pressure medicines you take before starting Sildenafil Citrate, and limit alcohol intake while using it.
